Identification, Date of Birth and Zip Code Checks

When entering data on a client's record, check the date that their information was last verified. 

  • If it's been several months since an ID check, ask the client if they have all necessary ID information with them. 
     

    • Clients may bring in their documents next time if there are NO comments on the record that the Client has been notified previously. Please insert a comment that “the Client will bring in ID/DOB and Zip code information next time” on the record with your initials and date.
       

    • If there are notes that the client has previously been asked to bring in their documents, and the client still has not brought documents with them, the client should not be served.  The client should be reminded again that documentation is required before service can be resumed.
       

  • ONLY Identifications with complete name, photo and date of birth are accepted as sufficient ID for Clients and Spouses.

    • Photocopies of the ID are acceptable

    • IDs from other countries or passports are acceptable if they have photo and date of birth.

    • School IDs without Date-of-Birth and Social Security Cards are not accepted.
       

  • In addition, the Client’s address and Zip Code may be checked with rent receipts, PG&E or telephone bills, release form/document (any facility/agency of incarceration), "Permission to reside" document (from Landlord/owner/renter), or a current piece of postmarked mail.

    • Addresses on Driver's Licenses are insufficient, since the State of California does not issue new IDs when people move.  We need verification that's current.

    • Current” in all cases means within one month of today’s date.
       

  • Proper ID for children includes Insurance card, Medi-Cal card, Immunization record, Birth Certificate, or any official document with name (First, Last) and Date of Birth.
     

  • Once the Client and their family ID/DOB and Zip code have been reviewed, (you may make appropriate changes to the record) you may serve the Client. Finally, change the “Current ID Check” section of the record (click on the field, then click on the pink button and today’s date will be entered automatically).
     

  • Please remember to remove any reminders for ID/DOB or zip code in the “comments” section of the record.
